I. GUIDELINES FOR CHICAGO CARES, INC. ("CCI")
VOLUNTEERS WHO WORK WITH CLIENTS
("Clients" shall mean any child, senior or other person you encounter at a CCI
project)
1. You are responsible for all Clients assigned to you. You should never stray
from the group with a client. For your own safety and that of the Client, you
should never be alone with a Client. Always stay with the group.
2. Two Volunteers must escort children to the bathroom and if you are on a
field trip and it is impractical for a large group to stay together, always
break into smaller groups of at least two Volunteers.
3. Please do not go up to any Clients' rooms, apartments or homes unless
specifically instructed to do so. You should remain in designated common areas
with the CCI group.
4. Never lift Clients out of their wheelchairs. Always ask for assistance from
a social service agency staff person.
5. Never take any Client in your car or drive them anywhere.
6. Each CCI project follows a specific program designed to ensure that we offer
optimal service to those in need and that you have a meaningful volunteer
experience. Each CCI project also has a definite beginning and end time. Any
activity undertaken by any Volunteer beyond the scope of the project outlined
to you by the CCI Team Coordinator or activity undertaken before or after the
designated hours of the CCI project is beyond the scope of the CCI project. CCI
does not recommend any such activities.
7. Strict safety rules must be followed during CCI projects, especially on
project field trips:

One Volunteer should be clearly designated as the head of the line and one as
the end of the line.

Keep Clients within crosswalks and away from the street or subway tracks.

Make sure you know where the group is meeting at the end of the project and do
not be late.

Always stay in a group with at least one other CCI Volunteer.
8. Do not purchase anything extra for Clients beyond what has been established
before the project.
9. Stay until the end of the project. It is disturbing (especially to the
children) if you leave early.
10. Explain all rules to the children (no fighting, no running off, etc.).
11. If the project involves eating, check with the community service agency
representative to ensure that the Client does not have any dietary
restrictions.
12. Transitional situations can be difficult for some Clients. It will help if
you discuss the game plan for the day with the Clients. In particular, be alert
at the end of the day when it is time to go home - some Clients (especially
children) may act out.
13. If you suspect that any Client is sick or may be the victim of abuse,
provide details confidentially to the Team Coordinator who will follow up with
our office. We will discuss it with the appropriate professionals.
14. You are acting as a role model to these Clients - please act accordingly
and do not use improper language around them.
15. Never pry into a Client's background or demean the Client's family or any
ethnic, religious or counter-culture group in any way.
16. If a Client acts out, please contact the CCI Team Coordinator or the
Community service agency staff member immediately. Do not take matters into
your own hands. It is crucial that you never physically touch a Client as a
means of discipline or demonstrate aggressive physical behavior in a playful
manner. Some of the Clients we work with have been physically or sexually
abused and any aggressive move even made in "fun" or "jest" may be taken
negatively.

provided a with a copy and that I have read the above "Guidelines for CCI
Volunteers Who Work With Clients", and agree to abide by these rules at all
times.

II. CCI VOLUNTEER INFORMED CONSENT AND RELEASE
In connection with my voluntary involvement in activities undertaken for, and
with the participation and support of CCI, a non-profit charitable
organization, I hereby agree, by signing the attached acknowledgement, for
myself, my heirs, assigns, representatives, executors and administrators to
release and discharge CCI, its officers and directors, employees, agents,
Volunteers, heirs, assigns and representatives, to the fullest extent permitted
by law, from all claims, demands and actions for injuries sustained to my
person and/or property as a result of my involvement in such activities,
whether or not resulting from negligence and I agree by signing the attached
acknowledgement to release and hold CCI, its officers and directors, employees,
agents, Volunteers, heirs, assigns and representatives harmless from any cause
or action, claim or suit arising therefrom. I hereby attest by signing the
attached acknowledgement that my attendance and involvement in such activities
is voluntary, that I am participating at my own risk, and that I have read the
foregoing terms and conditions of this release. I grant full permission for
organizers to use photographs of me and quotations from me in legitimate
accounts and promotions of these activities.

III. CHILD ABUSE AND MALTREATMENT BACKGROUND INFORMATION
By signing the attached ackowledgement, I hereby declare under the penalty of
perjury, that I have never been charged and /or convicted of any crime of
violence, child abuse neglect or endangerment, child pornography, child
abduction, kidnapping, rape or any sexual offense, nor have I ever been ordered
by a court to receive psychiatric or psychological treatment in connection
therewith.

IV. Volunteer Eligibility
By agreeing to become a CCI volunteer and by signing the attached
acknowledgement, I agree and acknowledge that CCI may terminate my volunteer
status at anytime for violation of these guidelines or for any reason.
ACKNOWLEDGEMENT

agreeing I hereby acknowledge and agree to comply with or be bound by the
provisions in Items I, II,III and IV collectively.
